Whilst questions 1-11, which deal with physical limitations, must be the result of a physical disability, the same does not apply to questions 12-17, in as much as there does not have to be an underlying mental health problem.
However, when looking to use the descriptor out of its normal context, the wording of the descriptor has to be taken into account, so for the Initiating Actions descriptor which corresponds to question 13, the phrase "due to impaired mental function" is included in each of the sub-descriptors. On this basis it would be unlikely that you would score points if the underlying cause is a physical one.
