Hi, has anyone noticed that on this question about going out, can you leave home on your own to go to places you dont know. There is no box to tick no ! only answers are usually, not very often and it varies. what are you supposed to tick if you cannot go out to places that you dont know ?

You can either add your own tick box for "never" or leave all of the boxes empty and add "see below". In either case you need to explain why you were unable to use the existing boxes.

Hi , thank you very much for answering that. I am dreading filling in the form and the thought of going for a medical. But I have been in touch with my doctor and asked if he could do a medical report for me and I have done the same with my ENT doctor and IBS doctor. I hope they help me if I get them.
