ESA Physical/ Mental health Question

I know this may sound like a stupid question, but I am just helping to fill my partner's ESA form for migration from IB to ESA, and on the physical part there are 2 questions "Communicating with people" and "Other people Communicating with you"; Now I know they are obviously talking about speech or Hearing difficulties Etc, but the main reason my partner has been on IB is because she has had Depression, Anxiety coupled with Social Phobia, which makes it very difficult for her to talk to people, so we have been concentrating on the Mental Health Section, and it can cause her extreme distress if put in any social, or otherwise situations where she is expected to communicate with people.
Would it be worth mentioning these difficulties in these sections, even though "Physically" she can actually talk to people ?
Also, is there still a period after the 4 weeks to get your form sent in ?, as I seem to remember reading it here somewhere that there was.

With regard the two descriptors for communicating; to score points you need to show he can make himself understood through speaking, writing, typing, or other means normally used; unaided by another person. for question 6
and
that he can understand communication by both verbal means (such as hearing or lip reading) and non-verbal means (such as reading 16 point print) using any aid it is reasonable to expect them to use; unaided by another person for question 7.
He will not score points if does not meet these basic criteria.
