just a quick question, im in process of income support to esa transfer. i havent needed to send sick notes in for years. when i send my esa form do i have to send with a sick note, thanks.

Could I suggest that lilliegez does send in supporting evidence though?
I fell into the trap of believing the transfer from IB to ESA did not need medical evidence as ATOS had told me in a phone call that they would contact my GP and call me for a medical assessment. This did not happen, so I am now in the tedious and stressful process of having to appeal.
I hasten to add that I did not use the benefits and work guides before sending in the ESA claim. Very foolish of me, and I am learning the hard way.

I agree. I'm in the same position, and had the decision 4 days ago. I scored ZERO points!!!! That's despite being diagnosed with a serious, degenerative, progressive disease 3 years ago, and my health being EXTREMELY worse in the last 4.5 years out of the 12 that I've been receiving IB.
Also, my medical was recorded, but that didn't stop the assessor twisting my words and missing out vital things I'd told him.
Please, please PLEASE send in the most detailed medical evidence you can obtain. I'm not an expert, and I only know a fraction of what the amazing people who run this site do. But, I do know what's currently happening to me. And I was naive enough to think just filling in the form well would be enough, as would be giving as much detail as possible during my medical. And I was led into a false sense of security at the medical, as the assessor seemed so kind and understanding.
