A couple of ESA question

When i add evidance such as a letter from my GP etc to my form should i write anywhere on the form what evidance i am including?

My GP letter states that i attend one college class a week, I use a dictaphone and my teacher writes for me aswell and i will have a writer in my exams should i put this in my form?

I was sent a paper form but filled out the online form. Do i need to send the paper form back as well or just the online form?

My boyfreind has agreed to ring the DWP in a couple of weeks to check they have recieved my form. How can i make sure that they tell him IE do i need to write on the form to give him permission?

Hi m89,
1.If you are to include extra pages to the PDF ESA50 print off, such as the G.P. letter that you mention, try and draw references to any such extra pages in the appropriate section of the ESA50, e.g. if the extra evidence was pertaining to Mobilisising limitations, you could write in the "Moving Around" section of the ESA50, " Please see further evidence of the limitations that I suffer with "Mobilsising" in the added page, Number xx".
You should put your Name, NI number and D.O.B. at the head of any extra pages that you may attach to the ESA50, it can also help if you number them 1 of 3, 2 of 3, 3 of 3, etc, etc.
Remember to only send copies of any extra evidence and keep the originals.
2. You should only include any supporting evidence that helps to show that you suffer from the limitations that may qualify you for an award of ESA.
3. You do not have to return the pink paper ESA50 that you received through the post.
4. As it is ATOS Healthcare that you will be returning your ESA50 to, it would be they who you would need to contact to see whether they had received it. Only after they have carried out your assessment whether as a paper exercise, or at a face to face assessment will they return your file to The DWP, who will then make a decision, taking into account the evidence and any recommendations made by ATOS.

They may ask some security questions such as your D.O.B. or NI Number, so it may be best if you are present if your boyfriend phones them.
Don't forget to take into account the XMAS post, etc, when returning the ESA50 to arrive with ATOS by the deadline given.
You could use Recorded signed for delivery, in which case you could use Royal Mail track and trace online to see whether it has been delivered.
