We do not live together so does it have to state that my partner is my partner?
Does it need to include his address and telephone number?

To be honest, I think it should have there name and address on it, but not their phone number, otherwise you risk it being treated with little weight if it is effectively anonymous.
The letter should state the relationship, how often the person sees you, and what if any care they provide, finally it should include a declaration that the information contained is true, with your partners signature.

You can but there will be questions raised as to whether it is still relevant to your current condition(s), so you will need to also show that
- - your condition is unchanged, or
- - it has worsened, and
- - that you have not adapted to your contiion in the interim

Hi sd,
In an ideal situation any medical evidence that you may be providing should be as current as possible.
Preferably, no older than 12 months old. However, if you have a 2 year old in depth report that is still pertinent to your current conditions and resulting limitations, you should state as much and, Yes, I would use the report. Besides anything else, it may help to show that you suffer from chronic, longstanding, progressive conditions. (if that is the case)
You also state that you are providing current evidence, which is a positive.
