I am having a lot of trouble finding information on this site.
Can someone please tell me where I can find
(a)A sample Esa50 form
(b)Details of the points awarded on each section of the form.

We don't hold a copy of the current ESA50 on the B&W site, but you can download a copy from the following link

The descriptors are explained in detail including there point vlaues, in our ESA claim guides
