Name of medical evidence form

If hospital doctors are contacted by the DWP for medical evidence for somebody's ESA claim, are they sent the same form (the ESA113) as is sent to GPs? I've been having trouble getting copies from the DWP of any DWP forms that my medical professionals filled out for me and plan to try asking the DWP for the specific forms by name.

I am not sure whether an : ESA 113 Questionnaire would be sent to a Hospital Doctor/Consultant, as it may be, with a G.P.
The likelyhood is that The DWP/ATOS may not have contacted your G.P. or Consultant, as mentioned above.
They are not compelled to contact a claimant's G.P. etc, and only tend to anyway where the evidence provided in the ESA50 or elsewhere, points to the fact that the claimant may have the limitations that would lead them to be given an award of SG.
This is why we advise that claimants should provide as much evidence as they can, including letters of support, etc from G.P.'s Consultants or any other relevant source.

There may also be an : ESA72 Form
There would definately be an ESA85 or an ESA85A, and an LT54.

The best way to find out whether your G.P./Consultant has been sent a forms to fill in regarding your ESA, such as an ESA113 is to ask them, it will be on record.
I think that it would be highly unlikely that they have contacted a Hospital Consultant.
