cancer legis. change DWP req more info?

I have received a req for more info where dwp say the change in legislation from 28/1/13 (that increases the categories of cancer Tx used to determine a persons ESA allowance)
They state that it may be beneficial to my claim
One side of the form I fill in and one side for my Dr to fill in.
No date to have it returned by
I am in N.I.
I am currently in the WRAG and get some DLA & H BEN
I have some brain and physical problems from a cerebral infarct nearly 20 yrs ago.
Having had Surgery in the past for cancer that has left me with some physical impairment separate to my original disability, I am unsure what to make of the form...
Anyone else get this form?

This is the first I have heard of this, there are changes to the Regulations in regard to cancer treatment being introduced at the end of January, that could affect ESA claimants in the WRAG. See paragraphs 7 to 10 of the following document.

I would suggest you complete the form and return as soon as reasonable.

Hi L,
You won't be able to upload a scanned copy to the site, due to security issues.
You could copy and paste it to a post, or e-mail a scanned attachment to Steve, the site owner, at :

You would obviously have to type the e-mail address into your e-mail as one continuous word, which I can't do here, as the site is protected by spambots.
