I too had the same problem.
The form was opening in Adobe Acrobat X. I went to the Adobe website, downloaded the latest version of Adobe Reader XI (11.0.01). I then opened the same file and it opened using Adobe Reader XI. You will notice that all tick boxes are there and shading for other replies.
I am able to save further additional information also.

Hi everyone
I have run a quick test and can confirm that version XI of reader will allow you to save changes made to the new ESA50, but only as a new document.
However, on my system I am still missing the additional tick boxes, and there is no solution to the problem where the amount of text exceeds the siz of the text box, although I am not surpised at the latter, as I suspect this is down to the definition of the form and not anything to do with Reader.
I had to manually download and install the XI version of Reader, it was not shown as an update on my Reader X install.

I have Acrobat XI on both my Mac and PC and both versions aren't showing the form as it should! There are missing tick boxes. 
Just rang the helpdesk and they confirm to me that it's the 'wrong form'!!!

I am still missing Yes/No and other tick boxes, too. I'm using Acrobat Reader XI (11.0.01). It seems there is only one tick box per question.

Can you email Steve about this call, as his ability to update the ESA Claim guides is dependant on the ESA50 form
