I'm sorry, my brain is obviously not working in full co-operation with my eyes today
A Med-3 is a Fit Note.
I have done a quick search for a Med 133 and cannot find any reference, is there any chance it could actually be an ESA113, which is a request to a GP for information from the DWP.
If not, can you explain the context as it might help us to find it.
