Yes it is fine to use photocopies of extracts from you medical records as evidence, along with your ESA50

You should try and draw reference to them within the ESA50, and go into detail about how and why the limitations that you suffer as a result of the substantiated medical conditions qualify you for an award of ESA.
Make sure that you put your name, NI Number and DOB at the top of any seperate pages that you are to include.
Make sure that you are using the right ESA Claims Guide for the ESA50 that you have received, which is explained here :
