Yes he could do this, as well as Tipexing out any that have been ticked in error.
Make it as clear as possible.

on page 6 of new esa50 it reads :--
tell us about all your hospital and clinic visits here
tell us how often you visit the hospital or clinic and why :---
are they asking about past visits, if yes then how far back ,
and what info should be entered on the form --- clinic name and hospital name and adress
advice please

I think you need to consider the importance of the visit as well as how often and how long ago.
So you might want to include a visit to a specialist clinic that occurred three years ago, but not include a regular visit to a clinic two years ago, where they just took your blood pressure.
I would probably go for everything in the last 12 months and then the more important ones moving backwards, but there is no hard and fast rule, so if you want to include more, then do so.

in last 12 months only one clinic was visited but before 2012 i use to attend three clinics, the other two refered me back to gp with recomended medication. and to continue taking that medication in long term. they have nothing more to offer to me.

Your case sounds just like my friends.
She will be putting almost what you said in that section. At least it shows that you were seen by a professional who made recommendations and shows there was not much else they could do.
