I have ME/CFS which is a fluctuating condition. I'm afraid that, if I ever feel well, I won't know whether it is a permanent recovery or only temporary. If I ever felt well enough to work would I be eligible for JSA? I have claimed IB, now ESA, for ten years.

In principle yes, as you were previously on IB I do not believe that you will meet the requirements for Contribution Based JSA as you will have no NI Contributions in the required years, however, you can still claim Income Related ESA, which will be dependant on your personal circumstances, which include household income, assets and savings and whether a partner works or not.
