In accounts of tragedy, suicide, despair, hardship, degradation, injustice and inhumanity, the report contains accounts of individuals’ experiences, MP’s concerns and charitable organisations’ criticisms of a system that is clearly still failing the most vulnerable and sick.

In a sincere opening statement, the author sets the scene for a moving 72 page document that includes, with little editorial interference, the true life experiences of those brave enough and strong enough to present their story in the public domain.
