question about esa113 form for dr was created by karen
I have just had a look at this form on another site and wondering if this is the standard form sent out to your dr?? I moved house and don't even know my dr, hes not really involved in my mental health issues and although he does have my diagnosis, i dont think he could go into detail about how it effects me and I have too many problems for him to list and I don't like the tick box part either! am I in my rights to go and see my dr and ask him to let me know if he does receive such a form?? or would i be entitled to a copy of it if I need to dispute anything on there? my previous dr knew me and actually wrote a brief letter which worked in my favour.

Happy New Year !! :)

The : ESA 113 Questionnaire is pretty basic.

These ESA113's are seldom sent to a claimant's G.P., and DWP/ATOS are under no obligation to send one.

It would be advisable to discuss the prospect of one being sent, with your G.P.

This is why we advise that members should send as much information as possible, including a letter of support if you can obtain one, with the ESA50, and not rely on ATOS/DWP contacting your HCP for further information.

You could ask your G.P. practice to contact you if an ESA113 was issued.

You would be entitled to a copy of an ESA113 to dispute the contents, if one was issued.

just thought i would chip in I recently had a form sent in to my GP I asked her if i could see it before it was sent, she was quite happy to let me see it before it was sent off and let me have a copy altho i had to pay for the copy.

This is actually a very common problem. Doctors often do not understand how their patients are affected by their diagnosis. This is particularly the case for mental health issues, although some doctors are more knowledgeable/understanding than others. As you are new to the area there can be no harm in making an appointment to meet your doctor and tell him a little about your health problems. You may wish to take a copy of the letter your old doctor wrote, in case it's not in your notes. You might wish to send a copy of this letter to Atos/DWP provided its not over 12 months old. If you have anyone else involved in your care such as a support worker, psychiatrist or social worker it would be beneficial to ask them to write a letter explaining your symptoms.
