Okay, the form has been filled in and despite a firm conviction that I've done it All Wrong, I'm getting ready to send it off, but there are just a couple more points, which I don't think are covered elsewhere.
I'm using the electronic version of the form - now I've printed it out it is, of course, no longer all bound together. I know that all the supplementary pages should have my name, DoB and NI number on them - but does that information need to go on every page of the official form as well, in case it comes adrift?
Do I staple it together (it's duplexed, so there are only some 10 actual pieces of paper)?
And lastly - I took a look, finally, at the address on the envelope to which it is to be sent. Not a DWP post box, but our local Atos centre - which is just up the road, closer than the nearest Post Office. There's a dispute in our household. One party maintains that Royal Mail lose things and that a short walk will deliver it the people themselves and we could have a receipt from them and know it got there today.
The other party says that Atos could simply deny the validity of a receipt - or even refuse to give us one, as soon as they've got their hands on the form. Whereas, with the Post Office, you have a proper proof of posting and guaranteed next day delivery anyway.
Do I need to add that the form has to be with Atos tomorrow?

Well done on getting the form completed, I know it is not an easy job!
I think it would be wise to make sure that your details are on every piece of paper that you send in and the pages should be numbered '1 of ' from the beginning of the form.
You might find that it would be difficult, if not impossible, to get a receipt for delivery from the ATOS office, they are often not really set up to handle mail. I would recommend the good old Royal Mail - if you are able to get it delivered in time!

I've put a request for a recorded assessment in the box where they ask you if you have anything you need to know about the assessment - is that enough for the moment? I'll put it in a separate letter when the appointment comes through.

It would be worth sending a separate letter to both DWP and ATOS asking for the recording, there is a sample letter in the ESA guide.
