Unsure about medical evidence

Please can you advise me on something as i am unsure whether to submit it or not. I have quite a severe condition which is recurring and can require emergency treatment in the hospital for flares. I have a care plan which I will be submitting which has details of this. In my last appointment with the consultant it was mentioned that I have this clearly defined care plan due to the nature of my symptoms and when to come into hospital. It then goes on to say that as my symptoms are relatively asymptomatic they don't need to see me for another year. I feel this is a bit concerning as this might be seen as it being fine. In fact, in means that I haven't had to have the intensive hospital treatment due to careful use of the care plan to manage my symptoms at home. Relatively I would feel is the key word.
My care plan details all the things I need to do at home to manage things at an early stage so it doesn't get to the stage of needing hospital treatment, so I thought it might be best to focus on that and explain that is still can be quite difficult and it's only due to support from family with doing this I'm able to manage and stay out of hospital.
Any advice on whether or not to submit it?

Hi Ob,
You should use any evidence that helps to highlight any of the Daily Living and Mobility Activities where you may need care or suffer limitations.
Try and tie in the evidence with the actual points scoring Descriptor for the applicable PIP Activities.
