How helpful is it to include a copy of a care plan for evidence? Also, part of my plan is handwritten, as my GP involved me in helping do it. Should that make any difference? Many thanks

If you have a combination of health conditions which affect each other, where would you explain this, would it be in the extra info section at the end of the form? For example, severe mental illness along with a fluctuating physical health condition requiring careful management? Many thanks

Yes, it could well be useful to submit your care-plan as evidence - particularly if we are talking about a PIP claim. As always, you will need to link these to the descriptors for the benefit.
It should not matter if part of the plan is handwritten, as long as it is legible.
People with several, interacting, conditions can find it hard to know where to explain about this. If the information does not tie in with the questions, it is entirely reasonable to include it with the extra information at the back of the claim form.
