PIP FORM and lack of recent medical evidence

Will receive PIP form within the next two weeks
or so.

Meeting new GP end of the week (he does not have my
medical records yet). Should I ask for a
medical report, summary to list medical conditions. based on
(see below) and send for PIP?

1. Should I take along snapshot of summary of diagnosis
from ex GP, along with photocopies of clinic letters from
hospitals consultants?

I am not under any consultants presently,
discharged back to GP's care. Unable to take meds, or
undergo invasive procedures.

Many of the letters state there are no medications, nor
will surgery help for those conditions.

I had an echo done last August to show on going
heart failure.

2. Can I send the above for PIP - some of the letters are not recent,
older than two years/

3. What if GP refuses to write short report to confirm list of
health conditions? What can I send to PIP in this case?

To try and answer your questions.

1. It can do no harm your taking some previous letters to your new GP, the worst that can happen is that they refuse to take them.

2. Yes you can, if they are older than two years then you also need to explain why they are still applicable.

3. I'm afraid that this is a possibility if they do not have access to your records. We would normally recommend that you request a copy of your records yourself, but if you are moving practices this may be an issue. You can try contacting the old practice to see if they can provide this service still.

Thank you Gordon. I have 9 boxes of my medical records
at home. I will send the old ones, even though, the diagnosis
still stands.

As for my GP, I realize he is not under any obligation to write a
report, even though, I have the list of diagnosis from my previous
GP and latest echocardiogram to show worsening heart failure.

I also seem to come across a lot of other people who do not have
recent medical evidence, because, they are referred back under the care
of their GPs or, hospital MDs do not write letters of support, not are clinic
letters explicit in terms of diagnosis, prognosis, and symptoms.

Unfortunately it's a common problem and likely to become more usual as time goes on and the pressures on GP surgeries increases!

Hi chafur 47

I read your post with interest as I too have Heart Failure and went through an operation to have a Pacemaker installed in me.

I don't know what part of the country you live in, but I suggest that you ask your GP if he/she will refer you to your community's Cardiac Nurse, who is a focussed specialiased individual on patients with long-term Cardiac conditions like yours is. She also has the authority to prescribe medication for you and works in conjunction with your GP upon referral; and has more of a personal contact and communication with you.

He/she may be able to help in constructing a letter of support for your PIP application, she will also have access to your previous medical records and is usually known by Cardiac Specialists in your regional hospitals and it could help you, especially with an unco-operative GP. Good luck.
