Hello Rosiebud
You would need to contact whoever is conducting your assessment (Maximus, etc) and explain your circumstances. The name of organisation etc should be on the letter inviting you f2f interview.
It would perhaps be a good idea to ask for the name of the person you contact, make a note of this, and then follow up your telephone request with a written one, giving the name of the person you spoke to on the phone.
You would need the support of your GP; if you ask him/her for a letter of support, then send that along to whichever organisation is assessing you.
Even though you request a home visit, it isn't guaranteed that you will get one; if you find that you are refused, perhaps the next step is to get your MP involved. Hopefully it won't come to that.
If you need further help, please get back to us; just in case you need to get back to us, bookmark or favourite us, then you can find us again easily. Good luck

Hi Rosiebud,
I hope your request for a home assessment is successful.
In future, please will you not post in capitals? This causes problems for people, such as those with visual impairments, who use assistive technology.
