Hi all, I am new to the forum and am just going through the DLA to PIP change, I am working through keeping a diary and getting the form filled in, slowly, I am registered blind with minimal sight, I use hearing aids, I also have fibromyalgia, menieres, BAM, diverticulitis and back problems, how on earth do I make them see that as indivials issues they are manageable but as they all have differing effects when they coalesce it is agony. I really am stuck and any advice would be great, Thank you

You need to be able to complete the PIP activities "reliably" and "on the majority of days" to not score points.
You can record your problems in completing an activity as being due to one specific condition, multiple conditions that limit you in different ways or as a combination of conditions that as a whole prevent you reliably completing the activity.
For each activity make a list of the problems you have with it and the underlying causes and then using the methodology suggest in the PIP Claim guide document why you can't complete it.
So as some examples;
You might say that Reading is limited due to your sight problems and Communicating due to your hearing ones, perhaps Preparing Food is impossible because you can't see what you are doing but also because of your Fibromyalgia you can stand in the kitchen.

Thank you Gordon, that is a great help, apologies for the delay in replying, I used your reply and the documents on the site to complete my PIP form, it is now done along with a diary and medical evidence and ready to be sent off by recorded delivery!
I have also had a f2f health assessment for ESA which was incredibly strange, the health professional was a Physiotherapist who only wanted to discuss my sight and hearing and sort of made assumptions regarding things I could or could not do, will wait and see the outcome of this in due course.
