I'm so sorry you are feeling so low; it really is bad to make you go through all this without any help. Hopefully your daughter will be able to help you with the next step.
Once you have your copy of the assessment, you and your daughter should go through it and pick out the places where you feel you should have scored points but didn't. Make a note of all these points, and when you begin to fill in your form make sure that you point out where your actual ability and what the assessor said was your ability don't agree. Don't be aggressive about it; just state quite clearly and calmly what the differences are.If you haven't enough space on the form to put your answers, then write your full answer to each question on a sheet of paper with your full name and national insurance number on it .
Make sure that you point out the problems you have because of the medications you are on; about your falling asleep during preparation of meals etc etc. Ensure that you point out that you don't go out the house because you physically and emotionally can't go out., and all the prompting in the world won't make you able to go out, Also tell them that you are unable to manage your own financial affairs, and rely on your parents to help you with this.
Is your daughter able to help you with writing out the form? If not perhaps you can contact the CAB and ask if they are able to help, or can suggest someone you can turn to to help you? DIAL can sometimes help, if you have an office local to you.Other members of the forum might be able to help you better with ideas of what to put on your form - if they have had the same experiences as you with the assessment and the award , and so on. When you have had a chance to look at your assessment form get back to us if you aren't sure where to go next and we will try to help.

The important thing is for the request to be made within the one month, if you don't then it introduces another set of problems that are best avoided if possible. So if you have time you can wait for the form, but if it does not arrive in time then send the request and say that you will provide more information once you have received the report.
