I would start by talking to your GP again, although they have already recommended a home assessment, they may have some views on why you are still being asked to attend at the Assessment Providers offices, it's possible that with a better understanding of the AP's thoughts about this they may be able to fill in the "gaps" with a new letter.
Failing that, contact your MP, their involvement can often result in a change of view without additional evidence being supplied.

Thank you so much for messaging me, my daughter rang the doctor, they said they would try and arrange transport for me but I'm so scared to go I don't like going out and especially trying to cope with strange people. I'm in a wheelchair most of my time. Yes ok I will try and contact my MP, thank you you're very kind.

The difficulty may lie in you going out sometimes which argues against you not being able to attend!
