As you know the extra information boxes on the ESA50 are ridiculously small and I was wondering how best to give further information for each question. I was going to tick the appropriate choice (e.g., yes, no, it varies) and then write the descriptor I feel is relevant in the extra information box and then further explain it on sheets of paper.
Is this a good idea or is there a better way to do it?
I think your idea for filling in the form is a good one. Only suggestion I would make is NOT to tick yes/no/it varies, just write in the box at the side that 'none of these applies, please see extra information on page..'. This way whoever looks at your application will be made aware that there are issues that they need to look at.
Make sure you put your name and national heath number at the top of each page, along with the page number (eg. page 1 of 3), and make sure you put the number of the page in the correct place in the application form (not being rude here, but it's very easy to forget to do this, and your response might be ignored)
Follow the information in the guides carefully and you will give yourself the best chance of receiving an acceptable award.
