Started to go through it and pages 1-9 give details of activities and descriptors in brief.
However from page 21 'Completing Questions 1 and 2' onwards it doesn't seem to follow the numbering given to the 'activities and descriptors in brief' i.e. page 22 Q3 preparing food is about Q1 and page 23 Q3a do you use an aid or appliance to prepare or cook a simple meal doesn't relate to Q3 Managing Therapy....
Is there another version that I should have downloaded? Confused. Help.

It follows the numbering on the PIP2 form

Question 1 is about the healthcare professionals that support you and Question 2 is about the health conditions that effect you and the Question 3 is for Preparing Food and is the first one that covers the PIP activities.

Wrote for PIP1 paper claim form 21st June 2016, stating why needed etc. Not received anything. Had reminder about DLA ending and deadline for claiming via telephone which is 4th July 2016.
Thought PIP1 would arrive but still hasn't.
Got hubby, who is disabled himself and dyslexic and suffering from Anxiety Disorder to ring the PIP claim line telephone number to inform of situation (in attempt to stop them suspending DLA claim).
He was told by person answering that they do not have a paper PIP1 claim and that she did not know what has happened to my letter. He asked to speak to a supervisor and the first adviser said she would go and find out what to do.
She came back and said that the supervisor had told her that there is no paper claim form and that if my husband cannot deal with the claim for me over the phone then I need to go and get another appointee to do it for me.
I wrote to the address given to me for writing for a Paper PIP1 and I have the Post Office proof of delivery (such as it is as the signature is just an initial).
What should I do now?
