You need to be detailed. You need to go through the different questions and say why you believe you still satisfy the criteria. You don't have to write out all your evidence again, but you do need to reference it in your replies and show why you disagree with the decision.

I'm sorry to hear about your difficulties with your PIP review. It is very demoralising when you suddenly receive zero points. First thing, do not give up!
If DWP award zero points after a previous award, it for them to show that an actual change of circumstances has occurred, not just the assumption of the assessor, who seems to have ignored everything you say. If I understand correctly, the assessment was recorded , do you have a copy if the recording? I appreciate it may be a challenging task, but have you listened to the recording? It is potentially possible that the assessor genuinely didn't understand. If you can rule that out then is there anything that shows why the assessor disagreed with everything you said? You could make a formal complaint if you wish.
I presume that you are using the CRMR1A form for your MR request. If it is easier, you can simply write a letter. I find the most successful way to write an MR is to set a out the background of your case including brief medical history and history if your PIP, including any DLA if you had this. Mention whether your health conditions are static, or have become worse.(I assume there has been no improvement.) It is important to stress that there us nothing in your nrdical history to show why the previous award can not continue.
Next write a few brief points about the faults with the assessment. Bullet points are great. Things like 'everything I said was contradicted by the assessor ' or 'the assessor has made sweeping assumptions about my medical history , which is not based on any evidence ' or 'the assessor appeared to understand and accept what I have said, but seems to have ignored everything I told him, without explaining why' are good ways of generic criticism of the assessment. Pick one or two of the most glaring examples and briefly include these. You could then add that you feel it is inappropriate for the assessor's comments to be reliable.
I would then have a look at each group 9f activities, (Descriptors). Go through these.You can say to the effect that 'previously I was awarded X points for...my disability has not improved as suggested...' then explain your limitation, ' I remain unable to...' and provide examples, perhaps using recent problems to indicate that you have ongoing difficulties.
Do this with all the descriptor activities. You may wish to make a more focused criticism of the assessor whilst explaining the descriptor. So for example, if the assessor claims you only take a small dose of simple medicine, explain that you have tried other options, and these didn't work, or gave side effects etc or state that you are on the maximum dose if this is correct.
If you have any medical evidence not sent in, please send it, or alternatively, if you sent this in before and it appears to have been ignored, resend and explain why you have resent It!
The PA4 form is the written version of the assessment report. Don't worry if you haven't got this. But depending how long you have before the deadline you may still have a chance to phone DWP and request this.
As with any recording, you can you through this and check what has been said and if you agree. You can still adopt the bullet point criticism I have suggested above. If you don't get the form, or haven't got the recording, you can simply concentrate on the comments made by the decision maker in the same way.
The guides give more information, so you find it helpful to have another look at these.
DWP do not make things easy, but you can receive your proper entitlement, but the important thing is not to give up!

It has been very demoralising, LL26 thank you for your detailed response this has helped me loads and I feel a little bit more confident not giving up with the DWP
