On page 25 of our PA 4 form the very last question the the assessor / decision maker answered-
The claimant has a mental intellectual or cognitive impairment and may need additional support to comply with future claim processes.
Yes / No box choice and they have ticked NO box

Sorry thought i was going off topic so needed to change the thread.
sorry to seem a bit dim but this specific question doesn't seem to have been answered anywhere we have looked on this or other posts.
we want as much info as possible to decide if we ask for a MR as its so draining even getting this far. could this last question on the last page be enough to challenge the obviously completely wrong answer? as my wife has not even completed this round off forms and assessments by herself and its caused her overwhelming mental distress

I'm afraid it isn't sufficient.
This question is about the help that a claimant might need at a future assessment, I'm not sure that anybody looks at it when the time arrives for a reassessment anyway.
You primary goal is to show that your wife cannot reliably and on the majority of days complete the PIP activities as this is the only way that she will receive an increased award. Dealing with the issues in the report is important, but won't lead to more points in itself.
