Welcome to the forum, you might want to have a look at the following FAQ which explains where everything is
Welcome to Benefits and Work
There is a link to the SSCS1 here
www.gov.uk/social-security-child-support...unal/appeal-tribunal
and the appeal process, along with the criteria against which you are being assessed is explained in our PIP guide
PIP help for claimants
You do not have to include a lot of information in order to submit the appeal request. Leave that until after you have received the evidence 'bundle' from DWP (this will include all the paperwork they hold associated with your claim) and you can then rebut their arguments.
The Tribunal panel will be more interested in hearing from you about how your condition/s affect your ability to carry out the activities linked to the PIP descriptors. They will be less interested in the assessor's report.
If you have further questions after you have read the PIP guide, we will do our best to help.
May I suggest that you bookmark/favourite this on your web browser now so that you can find it easily in future? This will allow you to return with further questions or comments about your PIP without having to start a new topic each time. We ask members to keep everything relating to the same claim in one topic as it helps us enormously - and I hope you will find it useful too

Malcolm wrote: Hi, Thank you for your response.. Do I have to contact DWP to lodge an appeal as the mandatory reconsideration letter says all i need to do is send information to the appeals dept. at and address in Bradford.. no mention of an sscs1 form or sending me an 'evidence bundle' How do i get that?
If you follow the links in Mrs H's post you will find the SSCS1 form and instructions on how to complete it.

The DWP should have included a pamphlet with their MR Decision Notice about appealing, if they have not done so then this would be their bad!
The Tribunal Service is a completely separate department from the DWP.
Once the appeal is accepted the Tribunal Service will notify the DWP and they will create a submission to the TS of how they have made their Decision and why they think it is right. You will receive a copy of this (their submission), you need to wait for this before you send your own submission so that you can make sure that you have addressed all of the issues that they raise.
You only need to provide basic reasons for your appeal on the SSCS1.
