There is no pamphlet with the mandatory reconsideration decision letter. So all I need to do for now is to fill in and send the SSCS1 form to Bradford within one month of the date on the letter and await a reply from them?

Yes, although I would start preparing your own submission now so that you are ahead of the game when the DWP bundle arrives.

Near the end of the MR there is a part that tells you about SSCS1 and how long you have to reply. Like Gordon has mentioned, you should get copies of medical records and any other evidence you have to support your claim and "photocopy" and send it away. The court will reply back to you and let you know that they have received the paperwork you have submitted pretty quickly (within 2 weeks).
