ESA 50 form for daughter

Yes its been a very difficult and sad time and still is she has difficulties but doesn't like admitting them. I am struggling to cope with all this and the form as I am not a psychologist or neurologist
Its hard to know if I am doing right or wrong and I am scared I will mess it up.
I have spent every day since it came trying to do it and finding it extremely hard to deal with and know what to write to explain her conditions and difficulties

It might help not to fill in the form directly. Make some notes for each activity and then convert these into some sentences, then in to paragraphs, etc. Try and break the process into small manageable amounts and do something completely different in between.
It can be really difficult being confronted in black and white by the difficulties that a loved one is having, so make sure you take time for yourself.

My daughter was diagnosed with Epilepsy and Polymicrogyria in 2011 and got ESA and also gets DLA for her epilepsy and needing to be accompanied supervised in unfamiliar places because of the risk of having a seizure she has Partial and complex partial seizures and now the last year secondary generalised because of the stress she been under
In the guides for ESA think its Q15 it says beware if you get DLA for supervision the esa form can be used to look again at your claim for DLA
Which Question would I answer regarding this is it Q8 in the Physical Getting around safely or Q 15 going out
This form is so confusing the descriptors are very vague and there most be thousands of disabled being affected and losing there sickness benefits because of it

There has to be a diagnosed mental health issue for a claimant to score points for questions 11-17, that doesn't mean you should not make any arguments that you have as to why your daughter meets the criteria but I would not rely on her scoring the points.

and depression
she has also since a breakdown in Aug 2015 been given a diagnosis of a emotionally unstable personality disorder and associated depressive illness this is after being in a domestic abusive and violent relationship for 5 years and then having to fignt for custody of her son for last yea rafter he took him for contact when she left him in Oct 2015 and him refusing to return him.
So as you can imagine her emotions are all over the place as its been a very difficult time to deal with

Gordan I have tried to save and print the ESA form and it wont save to computer or print
I have adobe reader DC will it work with this or not I
I am struggling and have made mistakes on the form they have sent to fill in
