The DM can choose what evidence they prefer it's also possible that they have either forgot or failed to document the other information.
The PIP Appeal guide has links to the SSCS1 form that you will need to complete and the Submissions guide has a complete example that you can use to help you fill it in, you do not need to provide detailed reasons at this stage for your request.
Once your appeal is accepted the DWP will prepare their submission to the hearing and send you a copy, whilst you should wait until you receive this to finish and submit your own, you should start to prepare your own case, an increased award will only be made by your showing that you score the extra points for one, so I would start by going back through your PIP2, the assessment report and the PIP Claim guide to see how you might show this is the case.

As on both original decision and the Mr only list "your how your disability affects you form" and the MR also says "extra information you sent us " but nothing specific and both refer heavily to the medical report and none of my evidence at all even thought there is an upto date letter from my GP proving all I have stated in my original form
Also on the medical assessment form the HP has not listed anything as also looked at evidence even though I sent in at least 20 sheets from drs gps specialists all that she says is "various evidence all before 2012?" But this is very wrong

These are matters that you can raise at an appeal, but remember, an appeal resets everything and it doesn't really matter if the DM looked at your extra evidence or not, the panel will look at everything with fresh eyes.

If you appeal then the DWP submission will include all of the documents included in the making of the Decision, if some are missing then you can submit them your self pointing out that they were sent to the DWP.
