Paperwork for daughters PIP form

My daughter is 20 and moving from long-term DLA onto PIP.
I've received the 'how it affects you' form today, and have until 7th May to complete (I'm her DWP appointee).
When we claimed DLA (child at that point) I had all her letters etc. from her consultant, all her hospital plans which I sent along with the application.
The problem I now have is that she is under Adult Mental Health Services nowadays and she has hardly seen her consultant, or indeed her care co-ordinator. I can count on one hand since she came out of Inpatient Care 14 months ago,how many times she has seen them. This is not the forum for discussing what I feel about them! There is certainly no written letters etc been given/sent to her, and I feel this may cause problems with the form... as you state that often the assessor won't even contact the names on first page.
I know the GP has done a report but this will be more about her physical health whereas the main issue is her mental health. The only thing concrete I have is her medication list.
Do you think I should do a SAR on her behalf for any written information from consultant and/or care-coordinator, and if so, which would be most useful. There have been a number of self-harm episodes which have required A&E and also Inpatient stays. If I write to the NHS Trust would that be sufficient (the Trust oversee's her current placement, consultant, care-coordinator)

A good starting point would be to check with her GP as to whether there are letters from her consultants on their files, if they are then you she can request them, this may be quicker than going to her Consultants and Care Co-ordinator. See

If the GP does not have copies then issuing a SAR to the local hospital about the A&E visits and any admissions. I would contact those who normally deal with her to see what is the best way for them to provide you with the information you need.
