As expected, the DWP has ignored my opinion and real medical evidence in favour of the "evidence" submitted by the nurse who made numerous errors and misrepresented my statements.
Do I still need to appeal within a month of receiving the mandatory reconsideration notice? I've been trying to read the Supreme Court judgement regarding ESA appeals/MR but it's all in legalese and I don't really understand it.
I also need information about gathering further medical evidence to help my case. I've got an appointment with my doctor on the 9th October(MR letter dated 18th September), so I will ask her if she can provide any. Is there anything else I need to know?

You still need to request an appeal within one month of the MR letter, otherwise you will need t make a Late appeal.
The PIP Appeal guide has links to the SSCS1 form that you will need to complete and the Submissions guide has a complete example that you can use to help you fill it in, you do not need to provide detailed reasons at this stage for your request.
Once your appeal is accepted the DWP will prepare their submission to the hearing and send you a copy, whilst you should wait until you receive this to finish and submit your own, you should start to prepare your own case, an increased award will only be made by your showing that you score the extra points for one, so I would start by going back through your PIP2, the assessment report and the PIP Claim guide to see how you might show this is the case.
An appeal is not always about providing new evidence, you may have submitted everything that is reasonably available. It's a further chance for you to show that you meet the criteria for the Descriptors that you believe you meet.
Consider requesting a copy of your medical records, these may hold letters and reports from any Consultants, etc. that you have seen.
