getting copies of the assessors reports was created by cazcomps
Last week I had a PIP assessment at home. I want to get a copy of the assessors report, but don't know what I need to ask for. Has the report got an official designation, or do I just ask for 'a copy of the assessment' or 'the assessors report'.
Also, who do I ask - the DWP or the company that did the assessment, in this case Capita.
On a similar question, when I made my initial claim I asked for a copy of everything the DWP had on my claim - I have been on DLA for sixteen years, indefinite for the last ten. They never sent this information to me despite me phoning a second time a week after the initial phonecall and asking again - when I was told they were organising it. Fortunately I have quite a lot of stuff I could send in my evidence pack.
Are the DWP legally obliged to send me this stuff when asked for, or can they just not bother? Am I likely to get the same non-response to a request for the assessors report? And if I have to ask for MR or go to appeal, will they then have to send me everything they use to make their decision?

Contact the DWP office dealing with your claim, asking for the assessment report should be sufficient but it's official name is a PA4.
There's no reason why the DWP should not provide the DLA information that you requested, but information is only held onsite for a certain time and then it is moved to offsite storage, which can delay the information being retrieved. There is a also a date retention policy that means that the DWP routinely destroy old documentation.
If you still want the information then you could make a Subject Access Request, this is a formal request and must be replied to by the DWP.
