I have been referred for the UC Assessment, however, I was supposed to fill the UC50 form in but it never arrived, and so I never sent the UC50 off. I received a letter asking me to return it a few weeks ago, but today I received an appointment date for the assessment in the post. Is it normal to be referred for the assessment without sending the UC50 back as they won't know how my condition affects my ability to work.
Or you can write a letter, I would use the same format as the form.
TAke it with you to the assessment and ask the assessor to read it before they start the interview and to include in your file for return to the DWP. I would also send a copy to the DWP office dealing with your claim.
