Hi. Just trying to cover all the bases on my wifes PIP review form.. if there is no change to a persons condition but they feel they were given the wrong points last time do you still need to fill in the 2 other parts of the question ie - ' Tell us how.....and 'Tell us about any changes to the help you need' etc etc. on the form it says to fill in every question...does that mean the second and third parts of the box if there is 'No change'? i was thinking of only writing 'no change' in the first box but explaining that my wif feels she was given the wrong points on her assessment and then on a separate piece of paper adding the reasons why. would this suffice? I appreciate all informed advice.

Hi Mathev

You need to complete the form as is it is a new claim based on your conditions and limitations as they are now, include as much evidence as you can even if it was submitted with your previous form as long as it is still relevant.

If you write no change in the box, then I would recomment you also write how your condition affects you as the assessor will not know you. I would not write about being given the wrong points on her previous assessment, if it was a current assessment then that would be different.

There have been some changes to the PIP criteria since you were last assessed so please make use of the latest version of our PIP Claim guide.

Our PIP Claim guide covers completing the AR1 Review form (1043 Review), remember, whatever the form says you are being assessed against the same criteria as before.

It is perfectly OK for you to type your answers and attach them as extra pages to the AR1, just make sure that each page has your full name and NINo at the top and that they are numbered, we recommend "page X of Y" so that it is easy to see if pages are missing. Make sure you clearly mark on the form where your answers are and on the relevant page that you are answering question X. If there has been no change in your circumstances tick the relevant box and then write below please see page ‘x’ of ‘y’, you do need to explain your condition even if it has not changed. If your condition has changed, then tick the relevant box and explain what has changed and how it affects you.

Attach the pages to the back of the form using a staple or make a hole in the top left corner and use a treasury tag, string or ribbon to the form. The pages need to be easily detached so that they can be scanned by the DWP.

Thankyou Gary. The PIP review form my wife received does not have tick boxs.On each activity itt just asks you to write if your condition has changed...to descibe how its changed,,how someone helps,and what help you need?
What I'm thinking of doing is writing ' No change' in the part of the question (without mentioning that we disagreed with her previous points score) then referring to the written pages attached that explain my wifes current condition in depth and what help she needs, The miniscule space they provide after each questio is not sufficient to write in detail. Then in the 2 bottom parts of each question on the form just refer them to the pages I have already attached that explains my wifes current situation. What do you think?

Hi Mathev

What you have written sounds good.

Make sure you write 'No change, see page x of y below', so the assessor knows where to look for the information.

The space on the review form is quite narrow but you do need to treat the review form as if you were completing your initial PIP application form.
