Can I resubmit an incomplete PIP 1003 form

Grateful for your advice please. A claim for PIP was submitted on my behalf last November by the Welfare Department of the Psychiatric Hospital where I was an inpatient.
At the time, I was extremely unwell, and was unable to engage with the PIP process; the Welfare Department assured me that they would submit a complete claim on my behalf.
Before I was discharged from hospital, I asked for a copy of the PIP1003 form. I am currently awaiting a telephone interview with Independent Assessment Services. As I’ve continued to remain unwell since discharge from hospital, I have only just looked at the submitted PIP form.
To my surprise, the Welfare Department have answered each question on the form by stating ‘see attached supporting information’. The attached form is entitled ‘Welfare Department PIP Medical questionnaire’. My concerns are that this questionnaire combines a number of questions on PIP1003 into one question, and then only gives short (2-3 sentences max) to each question. Having read your PIP guide, I am very concerned that the questionnaire is not providing the level of detail or accuracy of response for me to stand the best chance of a successful PIP claim.
So, my question is, can I submit additional information to DWP at this point (ie a completed PIP1003, drawing upon the Welfare Department’s questionnaire), or would I have to start a new PIP claim ?

Hi NickTe and welcome to forums.

Yes, you can submit additional information, just say that you want it added to your current claim.

I can understand your anxiety, but against that, an appliction submitted by a psychiatric dept on your behalf is going to carry a lot of weight, so I would be reasonably hopefully.

Thank you for your advice - really helpful.

Can I ask a related question - I have been diagnosed with anxiety and depression for 27 years +. But, things have got much worse in thee past 5 years, and especially the last year in terms of self/harm attempted suicide.

I have been in hospital 3 times in the past two years. Each time, the same psychiatrist insisted I had A & D. Then, when checking my PIP1003 form yesterday, I was amazed to see under Q1 that he had written 'Recurrent depressive disorder. Traits of Emotionally Unstable Personality Disorder'. At no time had he mentioned EUPD to me during my last admission or discharge.

So, my question is whether 'traits of EUPD' amounts to a diagnosis of EUPD or not. I suspect I know what you'll say, but still wanted to ask !

I am not medically qualified to answer your question. Sometimes specialist do not give a full picture to their patients as it could undermine their treatment. My answer to your question is to ask your psychiatrist to explain what they meant by 'Traits' of EUPD.

Symptoms in borderline personality disorder occur in 4 domains: affectivity, interpersonal functioning, impulse control and cognitive. The diagnosis requires that at least 5 of 9 specific criteria be met.
