New pip form? Fab 2021 was created by Mjef
Hi,
By telephone were advised a pip form had been sent to previous address and that a pip2 form was being sent to complete to the correct address.
Upon receipt is was totally different to that in the pip advice, and each section had the same 3 questions:-
Tell us if something has changed and approximately when

Tell us how you manage this activity now, including the use of any aids that you need

Tell us about any changes to the help you need or the help you get from another person

How should these be answered because they are so open and replies can be translated however they want them to sound, and after spending a week tweaking the reply using the pip guide and webcast, this format has changed everything!

Hello Mjef,

Your scream at the end of your post made me smile. I know just the feeling! All very frustrating for you.

But your efforts are not all in vain. Have a look at page 104 onwards of our PIP guide, and that has information about the form you have been sent. You will see that we advise people to give more information than just that which is asked for, so use our guide and your notes and go for it!

Ok, thanks, missed that.

Just a another general point to add, how do we reply to the questions as obviously without reading and writing skills the claimant is not filling the form in, just signing,

There is nowhere to say that the form is completed by someone else. (carer) so should it be in the first person.

Hi Meg,

I think there are two ways of doing the form, and I have used both. You can either write it all in the third person ‘John has difficulty etc etc’ or what I more normally do is write in the first person ‘I have difficulty etc etc’, but then include a statement at the end saying that I have read and explained the form to John and recorded what he told me. I have also read what I have written to him and he agrees this is correct.’

If you don’t have room on the form for this, or anything else, you can add a supplementary piece of paper. Just make sure you refer to it somewhere on the form and put the person’s name and NI number on each sheet.

Hi Meg,
would it be possible to get hold of blank new 2021 form, can you scan it at all. I am in the process of preparing an anticipated application for a relative who may soon face the ordeal of being transfered to PIP from Disability. I have found this site very useful but it is all a bit scary knowing what is about to come.

Hello sc,

The form which Meg was talking about was the form which the DWP sometimes send when you have been in receipt of PIP and they are doing a period review of the award.

I would expect your relative to be asked to fill in a PIP2 form, which is the form on which you make your first application for PIP. As yet, this is not available electronically, but there is a copy of it
here

Obviously, this is a link outside of our site, so we can't guarantee that it is accurate and up to date, but I scanned down it and it looked about right to me. It will certainly give you an idea of what the dreaded form looks like.

I can understand your curiousity about the form, but to be honest the best way you can prepare is to read our PIP guide and think about how what you know about the difficulties your relative experience relates to what you see in those guides.

I commend you for being willing to help in this manner. Your relative is very fortunate!
