My current DLA award officially ended in July 2020, but I still haven't received any notification that I need to apply for PIP. I would normally leave matters until DWP get round to contacting me, but I have now been diagnosed with a further medical condition and I'm also (hopefully) about to move to the other end of the country to nearer to family. Can anyone please advise if it would be advisable to contact DWP now or if I should just wait for them to get in touch?

Hello Moonraker52,
Assuming that you have stopped receiving DLA payments from the DWP that means that your previous award has stopped and not been reviewed and/or renewed, and I would not now expect them to contact you at all. So you are right, you need to ring the DWP to start a PIP application. Any PIP award will be backdated to the day you make the phone call.
The number you need to ring is 0800 917 2222, and simply explain that you wish to apply for PIP. The whole phone call can take a while, since you can have to hold for up to an hour for the call to be answered, and then the call itself can take about 10 mins.
You will need
your contact details, for example telephone number
your date of birth
your National Insurance number - this is on letters about tax, pensions and benefits
your bank or building society account number and sort code
your doctor or health worker’s name, address and telephone number
dates and addresses for any time you’ve spent abroad, in a care home or hospital
Once you have made that phone call it is time to start reading our Guides for PIP
Help for Claimants - PIP
You do have to be an individual member of this site to gain access to them, but I would be amazed if you don't think they are worth every penny of individual membership.
You will receive a form called PIP2 in the post from the DWP which you have a month to complete.

Sorry, I should have made it clear that I am still getting DLA - I'm uncertain whether its best to wait for the DWP to contact me or make a new PIP application myself as I've been diagnosed with a further medical condition.

That makes it all a lot trickier Moonraker.
Normally I would say find a local adviser who could consider your current award and what you might potentially get on PIP with the affects of your additional condition. Assuming that option is not available to you I can only suggest that you read our PIP guides and consider what you might be awarded and then make the comparison yourself.
PIP guides
DLA is assessed on different criteria to PIP and there are winners and losers when moving from one to the other. There is the potential for losing your DLA and getting no, or a smaller PIP award. To give you advise beyond that generic advise is unfortunately beyond the scope of this forum.
