Hi Toks,
I'm not quite sure what you are asking. Do you mean a care plan, to see what physical assistance could be given to help look after someone and its cost, or do you mean help to fill out a PIP form?
I'm afraid we don't offer advice about care plans. The members guides offer assistance as to how to fill PIP forms.

I apologise for not being able to help your query.

Hi Toks

Claimants usually get care plans from the people who already provide them with care. For example, someone who has a mental health problem may have a care plan written by their care coordinator. Someone who has physical problems and has contact with district nurses might have a care plan. As LL26 says - this is not something we can advise you on.

Hi, Toks

I don't know if this is any help, but if you are receiving care or support from your Local Authority, then they are under a legal duty to have carried out a needs assessment, and, if you are assessed as being eligible for the care and support, then they must also complete a care and support plan.
Many local authorities don't routinely give you these plans, although they have a legal duty to do so.
If you're receiving the support, then your social worker will give them to you if you request them.
They might be useful evidence to send for the PIP, although they are often incomplete.
I think a private report will not be taken very seriously by the assessor, although if the person is a professional therapist/healthcare worker, they ought to be.
