hi, When i rang to ask for my Wifes PA4 form the kind lady said i want to point out that it isnt a final report and that once it has been read it could be changed. can i ask is there any one who has receaved there PA4 form and been awarded PIP only to be told you are not entiteld to it after the DLA has looked at it. again sorry for the spelling /Gramma i have had a stroke .

Hi Silverfox 7

You have been wrongly advised, for PIP assessments you can contact DWP and request a copy of the assessors report (PA4) within 48 hrs of the assessment, we have been hearing on the forum that it has taken a bit longer for the assessments to be up loaded due to Covid.

What you need to do is phone up again, hopefully you will speak to a different person and request a copy of the assessors report. If you still have no luck ask to speak to a manager.

We do get forum members who are told the same as you that they cannot get a copy of the assessors report until a decision is made. This only applies when requesting a copy of a ESA assessment report and not a PIP assessment report, it is a training error with new workers at the DWP.

The decision maker looks at the assessors report when making a decision, they do not alter what the assessor has written, at times they do ignore what the assessor has written and come to their own decision.

hi Gary,
I am sorry I havent made my self clear. what i have said above is true, but forgot to say they did send out the PA4 form. which I now have. what I was meaning to say is she told us not to take the PA4 form as the final decision as once the DLA dept see it they can change the assesors decision. I was wondering if any has recieved the PA4 and had it changed.

I have heard of the Decision Maker being more generous than the Assessor's recommendations, but I have not met the opposite.

Dear Silverfox7, do you agree with the assessor's report? If there are serious things wrong with it, if you have not yet had the decision, you ARE allowed to write in to say so. If there are errors, omissions, or twistings in it, it is well worth writing in ASAP.

The right approach is to go through descriptor [activity] by descriptor, referring to your evidence you sent in using very neutral, factual, non-blaming wording.
If needed you can add extra evidence. Eg for our daughter the assessor claimed her pain medication was 'moderate' ie downgrading the pain. But I sent in the NHS page about her Tramadol to prove this wrong. She got Enhanced on both but would not have got this on the way the report was written. So you could say it was changed for the better - not the report, but the result.

thank you for your reply and to every one else replies. in the PA4 form my Wife has been granted enhanced rate in both so we are delighted with that but dont want to get to excited if the DLA disagree with it hence my questions. thank you again stay safe everyone.
