Good Morning LL26,
Thank you so much for your reply, I just called DWP (PIP Dept) and requested the (PA) form, I was told it should arrive int eh next 8 days, My pip letter dated 31st march 2021. I have the following disabilities
1) Lumber spine Stenosis since 2000, found out in 2013 after a MRI Scan it was congenital
2) Bi laterial Sciatica since 2000
3) Double Incontinence since 2011
4) idiopathic Blackouts since 2003
5) Colitis since 2000
6) Vestibuler Migraines since 2016
7) Social Anxiety since 2010
All of these combined has been a living nightmare for me, so thank you so much for trying to help me, I really appreciate any type of help, I only see a consultant for the Vestibular Migraines and a Bladder nurse every year for follow up and any changes to my Tena and anal plugs. I am shaking as I write this because I am so frightened and scared. The system is winning at the moment.

Hi Calesha
Only you can decide whether you are in the right position to take on an MR, but as LL26 says, there is only a small risk to you losing your mobility. Like LL26, I have only heard it happening to two people and their cases were very unusual.
We're not assessors, so we can't say categorically what you should be entitled to, but if it was my claim, I would certainly want to know why my points had been decreased when my conditions had deteriorated, and why I was not being awarded any points for care for issues like double incontinent.

Thank you very much for your advice, I'm awaiting the PA4 form which I was told would take 8 days in the meanwhile I will do some navigation around the forum, and request the MR form, and look at the descriptors and challenge any discrepancies.

I know this sounds silly, I've downloaded the MR form (CRMR1) do I need to let DWP I am submitting it,or just complete it when I've gone through the PA4 issues?

I usually just send the completed form off by recorded delivery, but for peace of mind once you have sent your form off contact DWP and let them know your have sent them the MR form (CRMR1 form).
Go through the PA4 report first before completing the MR form. I usually use a highlighter to highlight any inaccuracies, once you have gone through the report you then set out why you are applying for a MR and go through each descriptor one at a time why you disagree with the assessor and try and match it with any supporting documents you may have sent in.
Understand that the assessor is offering an opinion when they say that you can do the PIP activities, tackling these head-on is unlikely to be successful, you need to show that the basis of that opinion is founded on incorrect information, so try and refer to your evidence to undermine what they have said.
Also, try and stick to issues that lead to your scoring points, I often hear members referring to the assessors spelling, as an example, whilst it may show a lack of professionalism, raising it as an issue isn't going to help you get an award.
Your primary task is to show that you meet the criteria, there are many reasons you may have failed, you need to address each of these but don't get bogged down in criticising the assessment report unless you can clearly show that it is incorrect, it is a lot easier to argue the facts of the situation;
Lastly, make sure that you understand the criteria that you are being assessed against you can put the best case forward that is possible but you won't score points if you do not meet the PIP Descriptors.
