To add to the excellent replies made previously...
Being able to work when you have disabilities is important and DWP should NOT penalise you for trying to live a normal life. However I wonder if your employer makes reasonable adjustments to allow you to work. Or perhaps he turns a blind eye to any awkward behaviour etc. If there is any special treatment/circumstances that allows you to work, then you shoukd definitely say so. This could indicate help you might need in terms of PIP activities.
PIP is awarded on what you can do for the majority of days. It won't matter if you can go a day or two without having any material effect from your disabilities. This means you don't need to worry about what happens all the time. (Or failing to score points because you have a better day.)
Similarly under the Repetition rule, you have to be able to do the various PIP activities to an acceptable standard as often as reasonably required throughout the whole day and potentially night as well if relevant. So, preparing a cooked meal, maybe this is reasonably required to be done 3 or 4 times a day. If you have incontinence or other toileting issues, you might need help 10, even 20 times a day? It will all depend on your health problems. Communication is something that could be reasonably required 24/7 or at least all the time throughout waking hours. If there is anytime when you can't do any particular PIP activity when you need ir it is reasonable to need to do it, then you should score points.
This means that if for instance, there are times throughout the day, possibly at the same time, but it could be at anytime when tics mean conversation is difficult or you talk gibberish etc then thus would indicate communication won't be done to an acceptable standard and should score points. It doesn't matter if the tics are unpredictable and occur at different times or amounts. The important thing is that the problems occur for what is more than a trivial time. This will apply to all PIP activities.
Think about how you complete the activities, acceptable standard can comprise all sorts of things. Think what would be the normal way of going about things. You mention swinging arms into people whilst walking, this could indicate non acceptable standard. Constant spilling food whilst eating, or dropping cutlery or plates is unlikely to be acceptable. Likewise dressing where shirt buttons are done up all higgledy piggledy again suggests it's not acceptable. Whist I appreciate that all of the examples I have provided may not apply to you, I have tried to show you the sort of things to consider, giving obvious examples.
Don't worry about the assessment. Firstly not everyone requires one. Secondly you can take a friend or relative with you to support you. But so what if you can't talk, or speak gibberish or whatever it will then prove that you have communication issues, and would be good evidence!
