Hi Gary
Having had time to go through the PA4 form in more detail, it is a complete horror show. I don't think there's one section where there aren't multiple inaccuracies or references to things that aren't true and it is going to take time to unpick this and complete the MR form in full. I'm feeling in turns very angry and very depressed about it all, particularly as there is a bank holiday the week before my form is due in so I have to factor in both that and the postal issues at the moment, therefore not leaving me much time.
Anyway, my query is - they continually refer to me going shopping when I know I told them I do all my shopping online, plus they mention me using taxis, getting lifts to go out, and meeting friends and family. How would I evidence that this is all incorrect? Would a letter from my mum who drives me to all my medical appointments, and who knows I don't go shopping or out socialising be helpful? I still live at home as I need the daily help so she is in a good place to know about my routine.

You can only state what is true and the inaccuracies in the report, it is not uncommon unfortunately.
Yes a support letter from your mother is a good idea, she needs to state what support she gives you and why she has to support you.
I had a client once who used taxis, but she could only use the same taxi driver and had done so for many years, so don't let that throw you.
Make sure you sent any correspondence by recorded delivery.

Sorry, I have been away from the boards for quite a while so I didn't see your comment. I had to take some time to recuperate after the mammoth 10 page rebuttal that I sent for the MR. It was due in a week ago & was received in plenty of time (I sent it recorded) but I haven't received any acknowledgement as yet. I did draw attention to the incorrect information & will see if this, along with the additional evidence, makes any difference.
