Very worried about my renewal form for PIP

Hi I previously wrote a post about Community Law missing off some of my health conditions on my PIP renewal form (it was completed over the phone brought to me in a rush to sign and sent off by them - I never had a chance to check it ) - the more Ive read the form the more Im concerned about what she has written or rather the lack of what she has written - is it possible to ask DWP to re do my form? Or would this be really silly suggestion - my husband wrote an accompanying letter saying how things affect me or what has deteriorated - on the form she has mostly written No change from previous award however on the phone I told her things had become worse - hence I was being referred to a new dept Neurology for further investigations .... Im literally so worried about the form that has been sent off Im sure I will have my renewal turned down :(

Firstly try not to worry! Am I right in presuming that you haven't had an assessment yet? If I am correct, then it is early days. What you could do is to write into DWP now and explain that the form was done over the phone, and if I am also correct you were probably a bit anxious and worried. (Maybe your health condition causes you to forget or not process things properly, or perhaps phone isn't your thing?) Explain any reasons why you didn't perform so well on the phone call and also if there is any reason why you didn't go through the form properly. Then write down all your medical issues, with the dates etc, and a short paragraph about the general effects of the condition or how they interact together and/or with any medicines that you take and should also list. Head your letter' Further Information' don't forget to include your name and NI Number and PIP Renewal on all pages.
Then go through the descriptors again, using the members guides to help if you need to. If what was written on the form is complete and correct you could simply say that the form is correct and move onto the next descriptor. Then you could say eg
" 'Dressing/Undressing' due to the arthritis in my hands I have lost grip in my fingers and thumbs. I can no longer manage buttons. All my clothes are pull on ones, and I wear velcro shoes, which do not need laces to be tied..." go through all the descriptors - you might just need to add a paragraph saying 'I forgot to say...'
If you have any new medical evidence say so, do mention the referral to Neurology - even if you haven't got the appointment done, or a report, say why you have been referred and what's being investigated, as again that will add some context.
If you do the letter now, given that DWP have got quite big delays at present, the chances are that your new letter will make it to the assessor and or decision maker in time for them to consider what you have said. You could always phone DWP to check the progress of your renewal at the time and warn them you are sending in further information.
Of course, I can't be certain whether this will work, but it might do. (There is a much stronger possibility of you losing points or being refused if DWP don't have the full picture!) So give it a try and see.
If the renewal is turned down, then you will need to go through the comments from the decision maker and counter them. It's a good idea to get a copy of the assessment form PA4 once the assessment is done, if you have one, however well or badly the assessment goes. (Phone in 2 or 3 days afterwards and ask for a copy - you are entitled to get this - and refer to it in any MR letter, if you need to write one.)
However, that seems to be very much a future problem, and of course something not to worry about t the moment. Take each step as it comes. Concentrate now on writing your letter, and filling in all the gaps that were missed from before. Send that in. (Make sure you keep copies of whatever you send.) Then it is simply a waiting game. You may not need to an MR, and your benefit may be renewed or even upgraded. If you do need to do an MR then you can refer back to the members guides, or seek help again from this forum.

Thank you for your reply LL26 - Ive just made a new post this evening about going out and agoraphobia - again all part of showing evidence but previously I was awarded 10 points for cannot undertake any journey and 10 points for cannot follow a route - If you see my post hopefully someone can answer it .... getting myself even more anxious
