Have I been sent the correct PIP2 form? was created by Linda73
My support worker rang for me to start a claim for PIP. We filled the PIP1 form in over the form and they said they were sending me the PIP2 form but nothing arrived. She rang again, they extended the return date and said they would send a new form out. Again nothing arrived. She rang a third time and this time I have received a load of loose sheets that are supposed to be the form. However, in the bottom left corner they all say PIP 2 June 2018 but on the sample version online they all say PIP 2 January 2021 so I am concerned if they have sent me an old version of the form. I don't even know if it counts as a proper form with it just being a load of loose pages. It says they have also sent me an information booklet to help me fill it in but they haven't. There was also no return envelope and the last page of the "form" is missing so I don't know where to return the form to. I don't know when the deadline for returning the form is either as the letter says two different dates for this
I would get my support worker to phone again but she has had a family emergency and has had to go away for a while, not sure when she will be back so I am without help for now. Do you know if this form can be used or if they will need to send me a newer version?

Hi Linda73
You can fill out the form they have sent you. Regarding the two dates, if you can, you should call the DWP to confirm when the form needs to be returned. You don't want to miss the deadline. Members have said the best time is to ring early in the morning.

Thank you for your reply. It turns out it doesn't matter though as just this afternoon it has all arrived by email. It says as a reasonable adjustment because of my health condition they are going to communicate with me by email and have emailed me the latest PIP2 form in pdf format, the return address and a deadline date of the 16th November. I haven't filled it in or printed it off yet but hopefully there will be no issues. I didn't know they did this

That is rare for them to communicate by email - so I'm so glad they have considered your health condition. Good luck with the form. Come back and ask any questions if you need help - especially as your worker is away. You can find information from page 128 to fill out the review form.
