Hello B&W
I presently have an ongoing PIP claim for the "daily living" Standard rate.
Now over the last two years another more serious health problem has come along.
It involves the heart and breathing. And i wish to make a new claim for these conditions
as they force myself now having to catch local bus only transport with my freedom pass.
(as I now live outside the London area)
or catch a taxi, which is obviously expensive. In the present climate from reading the DWP
total chaos could you advise me how to make this new claim, and it's expected process time.
How and from whom do I get an application form ? etc.

You can report a change of circumstances to PIP which should trigger a review form, just bear in mind they will re look at your whole current award including length of award.

Hi tmato2001,
If you believe that your health conditions has become worse, with a view to gaining more benefit, such as a Mobility component, then you need to phone DWP and tell them your health has become worse. It is likely that they will send out a c form for you to fill in. It is likely that yoy may have to have another assessment too.
You do not need to make a new claim- if DWP agree that your health is worse and this fits within the descriptor activities then they will award additional points which can then increase your benefit amount on your existing claim.
Have another look at the Members Guides before you call DWP. Make sure that the changes in your health will comprise additional points. So, for an example, if you already get 2 points for needing help with putting on lower body clothing - (descriptor 6d) If you were awarded this because eg you could only bend to shin level so needed help to put on shoes, but now due to increasing back problems you need help putting on trousers as well as shoes, this won't provide any more points.
Likewise if your walking ability was 150m, but now is 75m- this is on paper a 50% reduction, but mobility descriptor 2b provides points for walking ability of 50-200m so despite a clear reduction in ability, you would still be assessed at the same points level!
Additionally, if you are minded to ask DWP to revise their decision to reflect worsening health, you may find it helpful to collect relevant medical evidence before you call DWP. (Or at least begin to obtain the evidence.) Getting doctor's letters etc can sometimes take a long time! Also you might find it helpful to create a disability diary. Note down what you can/can't do, or where you needed help throughout the day. List whether you felt tired or breathless did you have any pain and at what level. Maybe fatigue set in so you couldn't complete an activity or it stopped you doing something else? Think about the time it took to do activities, (obviously having in mind the descriptor activities set out on the Guides.) Think also about whether an impartial onlooker would think you had made a reasonable attempt at completing the activity. (Did you reach an acceptable standard?) Finally think about any times where you hurt yourself or might have done so, eg falls, stumbles, knives or crockery slipping from your hands. (Safety.) If you went out, how far did you walk, and were you able to reach your destination on foot or car successfully, if not why not. Note down what went well, and why, maybe you had a set of better days?
Having a diary over a week will help you easily work out what help you need, and where you struggle, this will help you fill out any form. Also you will have a contemporaneous record of your current level of disability which you can use as a benchmark for later renewals or an appeal if this was needed.
I don't know it will take for DWP to revise its decision - I'm afraid it is a bit of a lottery! However, the day you call DWP to begin the revision process will be the day when benefit is backdated to, if DWP agree to increase the award.

I had a feeling that the best way to sort out any claim, was to keep a daily diary. Also to provide very full answers to each question. Before I actually retired, upon making my very last claim for ESA, I was under the care of a gentleman from 'mind', who was very helpful when I made my last claim. This man's work at 'mind' was to help people with anything to do with the DWP. He advised me to make long answers and even to repeat them on relevant questions plus new information on that particular question.
I'm making this new claim because of two very important actions taken by this useless new clueless labour Government. 1). Both awards for the heating payments were not given this year, and put my financial situation in tatters. And 2). Because I now suffer from heart trouble and shortness of breath making walking difficult. On the days I do go out, for any reason, I have to use whatever transport I can that will be the cheapest. This is why a diary comes in use. Transport times, distance of the place I have to visit, and the time I have to return, all come into play.
All this has to be considered whenever I must travel out. So I'll do what you recommend and call the DWP in the first instance.
