ESA to UC form, treatments was created by AC
Thank you for your previous help.

When the form asks for medical treatments does this mean to list all prescribed medications I take or use. I use a CPAP machine, would this be a treatment?

Or is this more for things like physiotherapy, B12 injections.

Many thanks

Hi AC

For the UC application form there is no need to list medical treatments if you are in the ESA Support Group.

Hi David,

Do you have a source for this advice? Has this come from the DWP? Thanks.

Hi Ant

Claimants in receipt of ESA and in the support group should be placed in the LCWRA Group without providing a fit note.

Initially claimants in support of ESA Support group were being told by their work coach that they had to provide a fit note, DWP sent out a circular informing work coaches this was not the case:

Relevant legislation if you were getting old-style employment and support allowance (ESA)
Reg 19(1), (4)(b) and (5)(b) of the Universal Credit (Transitional Provisions) Regulations 2014, which provides that a UC claimant who was entitled to ESA immediately before claiming UC and who had been, in respect of ESA, determined to have LCWRA or is treated as having LCWRA (ie, had been placed in the ESA support group), must be treated as having LCWRA from the outset of their UC award and the LCWRA element must be included in their UC from the outset.
