ESA support group migrating to Universal credit

Hi, Please can someone advice. I filled in online form migration from ESA support group to Universal Credit.
I found it very difficult very overwhelming. I forgot to mention health conditions. I spoke to agent from DWP with help from C.A.B

I told agent I forgot to mention non epileptic attacks I had put dissociative seizures instead as this is another name for these type of seizures and I forgot to mention rhumatoid arthiritus.

Agent asked was rumatoid arthiritus a new condition I said no, he asked what day and year was I diagnosed, I remembered year but could'nt remember the exact day panicked I had to guess day.

Looked at my journal agent has written my name and that I have a new condition rhumatoid arthiritus that stops me working and the day and year started he has not mentioned non epileptic attacks.


Feeling very overwhelmed I have a ID verification at Jobcentre plus I 'm worried I won't be believed due to what agent has written condition is not new he didn't listen I am migrating from ESA support group to Universal I also have fibromyalgia, face and mouth pain, complex PTSD, anxiety, depression. Sorry so long really struggling with this.

You might want to have a look at the following FAQ which explains where everything is; www.benefitsandwork.co.uk/guides-for-claimants/faq/forum.

If you come across any information that is incorrect, put a correction notice on your journal, if you do not know a day just say you don't know, making up a day is the worse thing you can do.

Make a list of your conditions and the approximate date they started, for when you see your work coach, that way you will not panic and forget them.
